The hospitality services of Mediciti are well organized to maintain the friendly and nurturing ambience of the hospital

Aims at providing patients and their attendees a comfortable, clean and homely environment, in addition to excellent, high quality healthcare services

Patient Relation Executives (PRE)

Patient relation executives coordinate between patients and staff from admission to discharge.

They make rounds along with the consultants and carry on the necessary networking required on case to case basis.

They interact with patients, members of their family and, the attending nurse to identify specific problems and carry out necessary remedial measures.

For patients to be admitted, the PRE explains the admission procedure in detail and explains the tariff structure. After discussing with the consultant and billing section the PRE provides the approximate estimate of the entire hospital stay.

For any cross reference advice by the consultant for the patient, the PRE creates an immediate access with that specialist and expedites the visit.

PREs also explain the Dos and Don’ts of the procedures to the patients/family and the necessary steps to be taken before and after the procedure.

They work in close association with the consultant/ sister incharge and patient.

Any complaint or suggestion given by the patient / family is forwarded by the PRE in duly filled feedback forms

PREs attend to any problems with maintenance or housekeeping and inform the concerned department. They take care of any bed transfers of patients and discharge requests.

PREs also coordinate with the billing section to make sure that attendants know about the daily update of their bill this helps in advance collection

Clinical Coordinators

This concept was started with the intention of taking best care of all patients from entry-to-exit. All the critical areas and other patient wards are allotted to the Clinical Coordinators to improve patient care in the hospital

Starting from the admission of a patient until discharge, they will help the patients in all aspects like coordinating between patient and doctor. Clarifying the problems, if any, concerned to billing, medication and in many other aspects

With this concept patients feel that there is someone to whom they can contact any time for any requirement without feeling that they were left in an unknown place.

Nursing Care

A hospital may be soundly organized, beautifully situated and well equipped, but if the nursing care is not of high quality then the hospital fail in its responsibility, says Jeen Bernarett. The nursing department is the backbone of any hospital. Mediciti’s main objective is to inculcate professionalism in nurses by equipping them with required knowledge and skills so that they provide revolutionized health care with warm affectionate touch.

Mediciti hires qualified nurses from its own School of Nursing situated at Shamir pet. This nursing school has been in operation since last 10 years. After finishing their 3 years of GNM course the nurses are absorbed into our hospital.

Total number of nurses is 200 including administrative nursing staff. In critical areas and ICU’s the ratio of nurses to patients is 1:1. Duty of all nurses is shuffled in all departments on rotation by enabling them to get full-fledged knowledge in various departments.

Nursing care towards patients is an important aspect in our hospital. Regular training classes are taken by the efficient faculty to train the nurses towards hygiene, handling critical and emergency patients, communicating with the patient and computer proficiency. This helps in standardizing the nursing care.

Precautionary vaccination is provided for the nursing staff. Since we are facilitating the nurses with a hostel and a separate dining hall, all the nurses are available at times of emergency.

1.Academic Programme: The hospital is ensuring implementation of the curriculum of DNB programme. The institute is ensuring that all necessary training facilities and infrastructure is provided for implementation of DNB curriculum in the specialty. The DNB training is a residence based training cum employment programme wherein the DNB resident discharges the duties of a Junior/Senior consultant in the concerned department and is devoting at least 8 to 10 hours per day in the department, further for teaching/training of DNB trainees everyday and sending a soft copy of the same to the NBE which is being updated on NBE's own website every month. The Hospital is sparing the accredited department / its faculty any other infrastructure for NBE CME programme, NBE workshop and examination as may be required by the NBE towards to running of DNB programme.

The Institute/hospital is networking with other accredited hospitals/institutions for joint academic programme for DNB students. The hospital is ensuring that DNB candidates and consultants are sent for mandatory NBE supported CME programme every year. The hospital is giving a copy of DNB curriculum, guidelines for accreditation, registration, fee, information bulletin and other guidelines circulated by NBE from time to time to DNB candidates. The hospital is ensuring that the DNB candidates and consultants contribute research article for NBE journals and bulletin.

2. A logbook of academic activities indicating a daily record of academic work, thesis protocol, and cases presented and procedures done under observation / independently are being maintained by candidate and supervised by the designated guide of candidate and countersigned by Director/ Medical Superintendent of the hospital.

3. Thesis Protocol: The hospital provides sufficient time to DNB candidate for their thesis work and provides all necessary resource for data collection.

4. Training in Basic Sciences: In addition to the training offered in the specialty, the hospital is also providing training in the basic sciences and allied subjects of the concerned specialty by way of a mandatory one month rotation posting each year in the hospital's laboratory so as to enable the DNB trainees to gain invaluable knowledge in subjects like Anatomy, Pathology, Histopathology, Biochemistry, Microbiology and Genetics.

5. Library facilities: The learning resources for the DNB trainee(s) in the hospital are as follows:-
  • Standard latest editions of Post Graduate text books in the subject.
  • Internet access to trainees with institutional subscription to e-libraries and journals (like DELNET, J – Gate etc.)
  • Journals in the subject ( at least one international and three national). The library is accessible on all days except national holidays and for at least 12 hours each day.
  • The complete and the latest set of NBE's DVD are available in library. The latest details of DVDs are updated on the NBE's website from time to time.

6. Teaching Beds: The hospital is earmarking 30% of the beds in the specialty as 'General (Teaching) beds'. General Beds are those earmarked beds whose patients are looked after by DNB trainee(s) under the supervision of Consultants. These beds are classified as General/free beds and are provided with unrestricted access for examination and supervised management by the DNB resident doctors.

Research

Phase ii clinical trial with RBx 7796 in patients with mild to moderate asthma.

Inflammation plays a key role in the pathogenesis of asthma. An increased understanding of this inflammation has lead to an emphasis on therapy with anti-inflammatory agents. The department of pulmonology is involved in a clinical trial in association with RANBAXY LABS LIMITED.

Early experiments with RBx 4638, the tris salt of RBx 4209 demonstrated protective activity in animal models of asthma based on bronchial hyper responsiveness. This activity has been confirmed using the sodium salt of RBx 4209, namely RBx 7796.

The ability of RBx 4638 to inhibit antigen induced bronchio-constriction in sheep (Ascaris scum antigen) was assessed. It was found to exhibit a dose dependent protective effect on airway reactivity.

Phase I clinical trials were conducted as a double blind randomized, placebo, controlled single dose study in healthy adult male subjects. Five dose levels, 50,100,200,400 and 800mg were evaluated. It was safe and well tolerated with minimal side effects like erythematous lesions, headache, dizziness and ventricular premature complexes.

Phase II clinical trial has been initiated at our hospital in patients of either sex in the age group of 18 to 60 years. They should have mild to moderate asthma as defined by an FEV, reversibility >= 12% baseline FEV, >= 60% of predicted normal. Patients who are unwilling to give informed consent, pregnant or lactating women, those with history compatible with severe asthma, patients with significant renal, respiratory, cardiac, gastro-intestinal, hepatic, endocrine or hematological disorders are excluded from the study.

Achalasia cardia

Achalasia cardia is not an uncommon cause of difficulty in swallowing but may be misdiagnosed as its diagnosis needs a high index of suspicion. Response to endoscopic therapy is usually dramatic and produces gratifying results. We present 2 cases of Achalasia cardia which were referred to the gastroenterology Department of MediCiti Hospitals in the last 2 months. One from a peripheral Hospital in Sanga Reddy.

Case 1

A 50 Yr male patient presented with dysphasia of 3 years duration associated with chest pain with no response to proton Pump Inhibitors & Prokinetics. Cardiac Evaluation was normal.

Case 2

A 20 Year old student with progressive difficulty in swallowing of 3 months duration with a similar clinical picture Upper GI endoscopy revealed a hugely dilated esophagus with stasis of food in the esophagus & gastritis.

Physical examination was unremarkable in both patients.

Barium Swallow confirmed: A dilated esophagus with a smooth bird break narrowing at the location of the tight non relaxing LES (Lower Esophageal Sphincter). They taken up for pneumatic dilatation after 24 hours of nil by antibiotics. The microvasive rigiflex pneumatic balloon dilator ( diameter 3 cm ) was used under endoscopic and fluoroscopic guidance and the balloon placed in the esophagus across the lower esophageal sphincter and inflated to 810 PSI for 1 ½ minutes.

Both Patients experienced immediate relief of dysphasia. They were discharged 1 day post pneumatic dilatation and were asymptomatic on the 2nd month of follow up (last week).

Discussion

Achalasia Cardia is the most commonly recognized motor disorder of the esophagus. The term Achalasia means "Failure to relax" and describes a cardinal feature of this disorder, a poorly relaxing LES. The annual incidence of Achalasia remains unknown although a viral cause has been postulated.

Abnormalities in both muscle and nerve components can be detected in this disease although the neural is lesion of primary importance. The most important path physiologic defect is a reduction in the number of NANC ( Non Adrenergic Non Cholinergic ) inhibitory ganglion cells.

Patients with Achalasia may present with chest pain and pulmonary symptoms and may be preventions of complications including esophagi is from stasis, aspiration pneumonitis and carcinoma. Palliative treatments include drugs, calcium channel blockers and endoscopic therapy including pneumatic dilation and myotomy.

Endoscopic and fluoroscopic assisted pneumatic dilatation causes forceful dilation to a diameter of approximately 3 cm to tear the circular muscle fibers of the LES and Causes reduction in lower esophageal spincter pressure. The advantages of endoscopic pneumatic dilatation are the brief period of discomfort, short hospital stay and relatively lesser expenditure.

Mega Multi Specialty Free Health Camp at Pulivendula in association with YSR Foundation.

Mediciti Hospitals is privileged to have an association with YSR Foundation, a prestigious social service organization which is also running a free hospitals at Pulivendula. The motto of our association is to serve the poor and the needy suffering from various diseases. As a first step a Mega Multispecialty Free Health Camp was jointly conducted at Pulivendula. An astonishing 15 super specialists on 14th and 15th of March 2005. Expensive diagnostic tests like ECG, ECHO, X-ray were done totally free of cost depending upon the requirement of patients.

A 55 member strong paramedical and administration staff of Mediciti Hospitals, 100 volunteers from National Academy of Construction, Pulivendula and 50 volunteers from sri sai Bhakta Samahjam, Pulivendula have rendered untiring services for both the days.All the patients were provided with free lunch for both the days. About one hundred and fifty patients, who required treatment, including by-pass surgery, were referred to Mediciti Hospitals. These patients were treated at subsidized rates or in some cases free of cost.

Mediciti Hospitals initiates Health Awareness Programs for Police Officials

A Health Awareness Program for police officials of cyberabad was conducted early this year by Mediciti Hospitals at Bharathiya Vidya Bhavan which was attended by 400 members belonging to different wings of the police department .

The program constituted a series of lectures by super specialists of Mediciti Hospitals and was attended by dignitaries like shri M. K. Singh – DCP – Law & Order.
